Genetic DNA Testing

Pharmazam offers multiple testing options including FDA Cleared at home tests, onsite testing by one of our nurses, or can even be done by your physician using our Pharmazam MD Product.

How to Order a Pharmazam Test Yourself

1. Fill out the Order A Collection Kit form on the Pharmazam website and our medical department will send you one of our own FDA cleared collection kits.

2. Download and setup the Pharmazam App on your smartphone and input your personal health information. This is a Pharmazam physician ordered test, so all information entered in the app is extremely secure and protected under all HIPAA laws.

3. Once you receive it, follow both buccal swab collection device instructions and then register it online at pharmazam.com/newtest or by clicking the "Register Your Test" button on the home page.

4. Then, send it back to Pharmazam using the U.S. Mail prepaid shipping label already attached to the box you received that contained the kit.

5. Once Pharmazam’s medical department receives the kit, it will be registered and one of our physicians will order the test and have it processed at a licensed and approved U.S. lab.

6. When the test is complete and released by our medical department, you will be notified by email that your genetic results have been automatically linked to your profile. If our medical department uncovers any serious issues they might contact you to help resolve them.

FDA Cleared Test Kit

This kit is ordered by our physician and shipped to patients.

Your kit contains 2 collection devices. Please use both. Do not eat, drink, smoke, chew tobacco or gum, or use oral care products (such as mouthwash, toothpaste and whitening strips) 30 minutes before this test.

First, rinse mouth with water right before the test. Next, carefully remove the swab from the packaging, and ensure that you do not touch the tip with your fingers, teeth or allow it to touch any other surface.



Step 1

Swab back and forth
20x inside each cheek
(between gums & inner cheek).



Step 2

Unscrew the
swab-cap and ensure
contents are not spilled.



Step 3

Screw the swab firmly inside
of the tube and then shake
it back and forth 10x.



Step 4


Affix the barcode labels to the swab tubes lengthwise and seal them closed in the biobag. Next, go online to www.pharmazam.com and register the kit. Then place the biobag and completed/signed registration form in the box. Finally, seal the box by removing the self adhesive tape strip and return it via US prepaid mail.



Step 5

Download the Pharmazam app (or register online at Pharmazam.com) and set up your profile.

FDA Registered Dry Buccal Swab

This device is included in test kits and used by nurses in the field

Do not eat, drink, smoke, chew tobacco, or use oral care products
(such as mouthwash, toothpaste and whitening strips) 30 minutes before this test.

Step 1

Open the bag and remove the plastic tube.

Step 2

Hold the swab by the plastic tube with one hand, and with your thumb press the top of the white cap to open.

Step 3

Holding the swab by the grip (white cap), remove the swab from the plastic tube and rub the inside of each cheek vigorously 20 times (between your gums and inner cheek).

Step 4

Place the swab back into the plastic tube and securely close it.

Step 5

Place into biohazard bag with the first COMPLETED wet swab (DNA Genotek-FDA Cleared) and follow Step 5 instructions for the return to Pharmazam.
